Multi-unit abutments solve the angulation problem in full arch implant cases. When implants are placed at divergent angles (as in All-on-4), multi-unit abutments redirect the screw access to a common path of insertion for the prosthesis.
Angulation Options
Standard multi-unit abutments are available in 0, 17, 30, and sometimes 45-degree angulations. The selection is based on the implant axis relative to the planned prosthetic screw access. The goal is to have all screw accesses emerge within the occlusal table, not on the buccal or lingual surface.
Key Selection Factors
- Implant angulation: Measure the angle between implant axis and prosthetic screw path.
- Abutment height: Select based on soft tissue thickness. Cuff height should equal tissue depth plus 0.5mm.
- Emergence profile: Convex profile for esthetic zones, concave for posterior.
- Platform: Standard vs narrow platform affects prosthetic space.
