Cantilever extensions are often necessary in full arch implant restorations, but they must be engineered, not guessed. This article explains the biomechanics of cantilever forces and how to design safe distal extensions.
Force Multiplication
A cantilever creates a Class I lever with the distal implant as the fulcrum. Every millimeter of additional cantilever increases the force on the distal implant by approximately 12-15%. A 10mm cantilever can generate forces 1.5x the occlusal load on the distal abutment.
Design Strategies
1. Minimize cantilever length — design the shortest functional cantilever possible.
2. Taper the framework — reduce cross-section gradually toward the cantilever tip.
3. Light occlusion — cantilever pontics should have light centric contacts and no excursive contacts.
4. Consider adding a distal implant — when cantilever exceeds safe limits, add an implant rather than risk failure.
